2019 National Care Transitions Awareness (NCTA) Day April 16, 2019

NCTA 2019 was a great success! We look forward to engaging with you in the future.

The annual National Care Transitions Awareness Day helped to increase awareness and action in the beneficiary and provider communities of the importance of safe and effective care transitions between all care settings.

The NCTA Day featured activities that expand awareness and action about the role of community partners, with a focus on providers, payers, and other business entities. The NCTA Day featured leaders and innovators in health care as well as engaging and interactive speaker sessions throughout the day.

CMS is focused on the beneficiary experience, and care transition is a key element of ensuring a successful road to wellness. Putting patients first starts with informing beneficiaries and their families, guiding their journey through recovery and incorporating the patient voice throughout the process of providing health care. These leaders are committed to improving care transitions.

Injuries, sudden illnesses, complications – transitioning care means having to expect the unexpected. Navigating health care requires a partnership: practitioners and caregivers anticipating patients’ needs and preparing them accordingly, and patients being informed and proactive consumers of health care services. These beneficiaries' experiences highlight how these partnerships are improving lives.
